The Alerts (CSV) report file contains various information such as the Patient's information, different systolic/diastolic alerts, different pulse alerts and different glucose alerts. It is possible to see how many alerts of each type occurred during the period selected. This report facilitates the tracking of different alerts set in place for the patient.
Instructions:
1. Log in to CareSimple.
2. Click on the Reports tab on the left side of the portal.

3. Click on + Generate New Report.
4. A new window will appear. Select the Alerts (CSV).

5. Pick the desired Date range.

6. Click on Submit to confirm.
7. The file is now being generated. The status will be Export in progress. You may have to wait a few minutes for the export to be complete (try to refresh the page as well).

8. After the file is generated, click on Download.

9. Open the downloaded file.

| MRN | Medical Record Number – a unique identifier for the patient. |
| DOB | Date of Birth of the patient. |
| Age | Patient's age in years. |
| Gender | Patient's gender. |
| Provider | The healthcare provider or physician associated with the patient. |
| Patient status | Current enrollment status (e.g., Enrolled, Archived, Referred, Paused). |
| Tags | Labels or flags indicating patient conditions or program participation (e.g., CHF, Oxygen Tank, Pilot). |
| Assignee | The care team member(s) assigned to the patient. |
| Date of First Measurement | The date when the patient first submitted a health measurement. |
| 3 or More Alerts | Indicates whether the patient had three or more alerts during the period (Yes/No). |

| # Measurements | Total number of measurements submitted by the patient. |
| # Diastolic Yellow Alerts | Mild concern level for diastolic blood pressure. |
