The Measurement (CSV) Report file contains patient information and the values of different measurements taken during the period selected. It is possible to see any modifications done and the time at which each measurement was taken.
Instructions:
1. Log in to CareSimple.
2. Click on the Reports tab on the left side of the portal.
3. Click on + Generate New Report.
4. A new window will appear. Select the Measurements (CSV).
5. Select the desired Date range.
6.Click on Submit to confirm.
7. The file is now being generated. The status will be Export in progress. You may have to wait a few minutes for the export to be complete (try to refresh the page as well).
8. After the file is generated, click on Download.
9. Open the downloaded file.

| Patient local measurement date | Timestamp of the measurement in the patient's local time. |
| Modified date GMT time | Timestamp when the record was last modified (GMT). |
| Patient local modified date | Last modification time in the patient's local time. |
| Server modified date GMT | Timestamp when the server last modified the record (GMT). |
| Patient local server modified date | Server modification time in the patient's local time. |
| Test ID | Identifier for the test session if multiple measurements taken together. |
| Modified by | User or system that modified the record. |
| Source | Device or method used to collect the measurement. |
| Deleted | Indicates if the record was marked as deleted (1 = deleted, 0 = active). |
| uid | Unique identifier for the patient. |
| First Name | Patient's first name. |
| Last Name | Patient's last name. |
| Gender | Patient's gender. |
| Age | Patient's age at the time of measurement. |
| MRN | Medical Record Number – a unique identifier for the patient. |
| Patient status | Current enrollment status of the patient. |
| Tags | Labels or flags indicating patient conditions or program participation. |
| Assignee | Care team member(s) assigned to the patient. |
