View and Create Custom Questionnaires
- Log in on CareSimple.
- Click on the Admin tab on the left side of the portal, and then on Questionnaires.
- In this section, you can view all the custom Questionnaires your Organization created and their information. You can also filter using the filter tool to filter by Status or Authors.
- To create a new Questionnaire, press on +Add Questionnaire.
- You can add a title and a description to your survey by using the heading of the page. You can do the same for each page of your questionnaire.
- To create your Questionnaires, four options are available:
- Rating Scale (1 to 5 stars)
- Checkboxes (Check one or multiple items)
- Dropdown (List with one item to choose)
- Yes or No
- Each of those options have the possibility to put a title (your question) and customize the possible answer your patients will have available to answer. To do this, click on them and customize the text.
- You can add or remove items by pressing on the buttons on their left side.
- You can put one or multiple questions on one page if you desire. You can also click and drag to move the questions around to make them appear in front or under a question or in another page.
- You can also duplicate a question, make it required to end the test or delete it by using the options available at the bottom of the question.
- Once done, save and publish your questionnaire. This will give you the option to assign it to patients.
Preview your questionnaire
- You can preview your new questionnaire by clicking on "Preview". This tool can help making sure that everything is in order before publishing it.
Add logics to your questionnaire.
This tool gives you the opportunity to alter the logic between the questions if some conditions are met. For example, it can help making questionnaires faster for people with no symptoms for a certain disease by reducing the amount of details they have to put in if there is no need for them to explain further.
- Click on "Logic". Click on "Add New Rule"
- Choose a question for which you want to add a condition.
- Choose a condition to meet for the action to happen. You can also add another condition if you desire.
- Choose the action to take if the condition is met.
- You can always remove conditions or actions by clicking on the delete icon.
- Click on "Done" to save the new rule.
- The new rule will then appear as shown below.
- You can edit a rule by clicking on it. You can also delete it by clicking on the delete icon.
