Add Dexcom Care Plan Template to a group
1. Select the group you wish to add the Dexcom authorization template.
2. The Dexcom Authorization template will appear as such
3. You can then add the template to individuals in the group.
Add Dexcom Care Plan Template to a patient
1. After adding the Dexcom Care Plan template to a group, you can add it to a patient. For this, log in to the CareSimple portal.
2. Select a patient and make sure they have a phone number in their Patient Information tab, as they will receive a SMS message for the confirmation.
3. In the Care Plan tab, click on the three dots and then on +Add from Template.
4. A new window will appear with a drop-down menu. Select the Dexcom Authorization Care Plan Template. Click on Continue to confirm.
5. After adding the Care Plan, the patient will receive an SMS message like this one with a link. They will also receive a message in the CareSimple app.
6. The patient will be redirected to the Dexcom log in page
7. After they log in, they will asked for permission to disclose and authorize the use of Dexcom for CareSimple purposes.
8. They will need to scroll down to reach the place designed for the electronic signature. To confirm, they will need to click on confirm.
9. Once the Authorization is complete, they will be directed back to the CareSimple page.
10. A confirmation will be sent to confirm the authorization
