View a Patient's Questionnaire Response

Instructions: 

1. Log in to CareSimple.

2. Select the patient you want on the Patients tab.

3. Click on the Questionnaires tab. 

4. A list of completed questionnaires is displayed (1). To view the results of a specific questionnaire (2), click on the questionnaire of interest. Adjust the dates desired. The questions and answers for this questionnaire will appear below.

5. Select the desired questionnaire and click on ANSWERS. 

6. Select the time frame desired for the graph by using the dropdown menu. It is possible to use the cursor to hover over the points to obtain the details (name of questionnaire, date. and time, score). All the questions are colour coded and can be found in the legend on the left side of the graph. 

7. Click on the question in the legend to exclude it from the graph. Click on it again to include it.

8. A score from 0-40 is given to certain questionnaires and is graphed under the "SCORE" tab.
