Managing Patient Statuses

Patient statuses refer to the state of the patient in the program journey. Some statuses are automatically updated while others require your input.

Onboarding

  1. Referred: Set automatically when patient is created on the platform but is waiting care plan or device setup. During this state care plan compliance alerts and task reminders are OFF and medical alerts are ON.
  2. Device(s) ordered: Set automatically if device(s) were ordered during the onboarding step but not shipped by CareSimple. During this state care plan compliance alerts and task reminders are OFF and medical alerts are ON.
  3. Device(s) shipped: Set automatically when the device(s) ordered for the patient have been shipped by CareSimple. During this state care plan compliance alerts and task reminders are OFF and medical alerts are ON.
  4. First reading transmitted: Set automatically when the patient has received their device(s) and took their first measurement. During this state care plan compliance alerts and task reminders are OFF and medical alerts are ON.

Enrolled

Enrolled: To be set manually when the patient has been successfully onboarded and is ready to start the program. This will turn ON the care plan compliance alerts and task reminders.

Paused

Paused: To be set manually when the patient is back in the hospital and therefore will not send more readings. This will turn OFF alerts, readings and reminders to prevent from being generated or received on the platform. This will also lock their mobile application account and show a message when logging in to get in contact with their Care Provider to reactivate their account (switching back to Enrolled).

Offboarding

To be removed: Set manually when the patient is ready to be offboarded from the program. During this state care plan compliance alerts and task reminders are OFF and medical alerts are ON.
