How to View/Edit a Patient's Care Program(s)

Instructions
1. Log in on CareSimple
2. In the Patient tab, select a patient.
3. In the Patient's chart, you can view in which program they are enrolled.
4. To edit the program, click on the Patient Information tab.
5. Click on the pencil icon in the upper corner of the Medical Information section.
6. Use the dropdown menu to select which program you want to associate with your patient. Click on Save to confirm.
Important: Per CMS rules, you cant select more than one program, but only one for monitoring. Each monitoring program can be paired with the CCM program. You can also select a Coaching Program.
7. The Patient is now part of the new program you chose for them.
