View Your Care Plan

Instructions:

1. Open the "CareSimple" app on your phone or tablet.

2. Click on "Profile".

3. Click on ''Care Plan''. 

4. Here, you will be able to view the care plan that was created for you. 

Under "Scheduled Tasks", you will be able to view tasks that you need to complete. You can scroll up/down to view all tasks. Each task has a time below it, letting you know when you will be asked to complete it.

Finally, in the section "Reference Ranges", you will be able to view the reference ranges set for the measurements you will take. Once you take a measurement and if reference ranges are set in your care plan, they will be displayed in the colours accordingly. You will be able to know if your measurements are "Critical High", "Moderate", "Normal", "Low", etc...
