Clinicians: View/Edit a Patient's Care Plan

Instructions: 

1. Log in to CareSimple.

2. On your dashboard, locate the patient whose care plan you want to view/edit.

3. Click on the three dots next to the patient's name. From the dropdown menu, click on "Profile". The profile will appear on the right-hand side.  

4. Press on the icon indicated below to view the patient's care plan. The care plan consists of six components: "Goals", "Reference Ranges", "Alerts", "Observations", "Messages" and "Questionnaires".

5. If you chose a care plan template for the patient when onboarding, you will be able to view/edit it here. If you did not choose a care plan, you can do so here by clicking on "Edit Care Plan", then "Add from Template +". From the dropdown, select which care plan template you want to add to the patient and press "Select" to save.

6. The first section of the care plan is "Goals". The "Hypertension" care plan we selected when onboarding does not include any goals. To add a goal for the patient, click on "Add +" under the "Goals" section. In the text field highlighted below, enter your desired goal. In order to save your changes, you must press "Apply to Patient". It will now be displayed on the care plan. 

7. The next section is "Reference Ranges". The "Hypertension" care plan we selected when onboarding included reference ranges for specific observations. You can choose to edit/delete the reference ranges or add your own reference ranges for other measurements.

8. To remove reference ranges for a measurement from the care plan, click the delete icon highlighted below. 

9. To edit the reference ranges from the template, click on the pencil icon highlighted below. On the pop-up, you have several options to allow you to fully customize the reference ranges and alerts. First, you can change the values set for reference ranges by clicking on the text field and entering your desired value. 

10. If you would like to remove a certain alert level, click on the delete icon highlighted below. 

11. To customize which alert levels you want to receive notifications for, press on "Alerts". You will be notified for those with a bell icon/selected on the left-hand side. 

12. You can also choose to start from scratch by pressing "Clear". You will now be able to fully customize the alerts and reference ranges as wanted. Press "Submit" to save your changes. 

13. The next section of the care plan is "Alerts". The "Hypertension" care plan we selected when onboarding includes smart alerts. Alerts are only available from care plan templates. The alerts you see below are for patient compliance, notifying you if the patient is not taking measurements. To delete an alert, press on the icon highlighted below. 

14. As you scroll down, the next section you will see is "Observations". The "Hypertension" care plan we selected when onboarding reminds the patient to take their blood pressure at 8:00AM and 3:00PM daily as well as input their steps at 6:00PM. To edit the observation, press on the pencil icon highlighted below. To delete the observation, press on the delete icon highlighted below. 

15. To add a new observation, press "Add +". On the pop-up, you can select the "Observation" type using the dropdown highlighted below. You can also customize the "Notification Content" by clicking on the text field or keep it as is. To change the date, click on the calendar icon and select your desired date. Below, you can change the time the patient will be asked to take the measurement. Click on the text field to enter in the time. If you would like to have a recurrence for the observation, you can do so by using the up/down arrows. You can customize the recurrence to daily, weekly or monthly. Lastly, click "Add" at the bottom of the pop-up to save. 

16. The next section of the care plan is "Messages". The "Hypertension" care plan we selected when onboarding does not include scheduled messages. If you would like to schedule a message to the patient, click "Add +". Click on the text field highlighted below and add your message. Next, click on the calendar icon to select the date. You can then change the time the patient will receive the message by clicking on the text field to enter in your desired time. If you would like to have a recurrence for the message, you can do so by using the up/down arrows. You can customize the recurrence to daily, weekly or monthly. Lastly, click "Add" at the bottom of the pop-up to save. 



17. The last section of the care plan is "Questionnaires". The "Hypertension" care plan we selected when onboarding does not include scheduled questionnaires. If you would like to schedule a questionnaire for the patient, click "Add +". First, select the questionnaire you would like to send from the dropdown highlighted below. If you would like to change the "Notification Content", click on the text field to make your changes. If you would like to send the questionnaire now and only once, click the toggle switch. Otherwise, below you have the option to complete fields for the date, time and recurrence. Click on the calendar icon to select the date. You can then change the time the patient will receive the questionnaire by clicking on the text field to enter in your desired time. If you would like to have a recurrence for the questionnaire, you can do so by using the up/down arrows. You can customize the recurrence to daily, weekly or monthly. Lastly, click "Add" at the bottom of the pop-up to save. 

18. In order to save all your changes, you must press "Apply to Patient" at the top.
